Term coverage is the purest kind of life insurance, with no costly savings features.

Term Life Insurance is a type of term coverage in which the death benefit amount decreases as you age. This is typically because there is less need for higher coverage because, for example, children become more self-sufficient and/or your mortgage may be paid off.  Accidental Death and Dismemberment (AD&D) is designed to provide additional coverage if you’re seriously injured or killed due to an accident. This policy combines these two valuable types of insurance coverage in one policy. However, you also have the choice to only purchase Term Life coverage.

Certificate Of Insurance

This information is only a brief description of the principal provisions and features of the Policy. The complete terms and conditions are set forth in the group policy issued by New York Life Insurance Company to Trustees of American College of Physicians, Inc. Insurance Trust. When you become insured, you will be sent a Certificate of Insurance summarizing your benefits under the Policy.

Eligibility

All ACP Members (including Associates, Medical Students, Physician Affiliates, Non-Physician Affiliates, Fellows and Masters) under age 75, may request coverage for themselves, their lawful spouse under age 75 and all unmarried dependent children ages 15 days through 25 years as a convenient “add-on” to any coverage you may have through work or on your own. A dependent who is also a member is only eligible for member coverage. If both the member and spouse are covered as members, only one may insure any eligible children. To become insured, individuals must provide satisfactory evidence of insurability and pay the required premium.

This coverage is available only for residents of the United States (except territories) and Puerto Rico.

Non-Dependent Family Members: Any eligible nondependent family members may also apply for coverage as long as they join ACP as an Associate Member. For membership information, please call ACP directly at 1-800-523-1546.

Group Rates:

The ACP Term Life Plan specifically targets the expanded coverage needs to ACP members. And that targeted audience was the key to your exclusively priced rates. The end result is a significant break for you: Negotiated rates…on solid protection…extended to the ACP eligible members.

Amounts Of Life Insurance:

ACP protection is also available when you enroll your lawful spouse under age 75 and your dependent children, ages 15 days-25.

Members: $10,000 up to $1,000,000 in $10,000 multiples. 
Spouse: $10,000 up to $500,000 in $10,000 multiples, not to exceed 50% of member’s coverage.
Child(ren): $5,000, for Children older than 6 months; $500 for children 15 days and older. (No coverage for children under 15 days)

Select the AD&D Insurance option if you want to double your life benefit amount. This provides an added safety net should you be seriously injured or killed in a covered accident. (See below for more details.)

The total amount of coverage an individual may have under all group life insurance policies underwritten by New York Life Insurance Company may not exceed $2,000,000. In addition, the total amount of coverage an individual may have under all policies issued by New York Life Insurance Company to the Trustees of the American College of Physicians, Inc. Insurance Trust may not exceed the maximum benefit option for any insured person.

 

Double Your Coverage with the AD&D Option

If requesting the Member Term Life Insurance, you can also request an equal amount of Accidental Death & Dismemberment Insurance (AD&D) for yourself. This option allows you to double your selected life benefit amount and provides an added safety net should you be seriously injured or killed in a covered accident. If your death is due to a covered accidental injury, the insurance will pay your life benefit amount plus your AD&D benefit, which doubles your life benefit amount. It can also pay a percentage of your amount if you are seriously injured and suffer a significant loss, as described below.

The injury must be directly and independently caused by an accident while coverage is in force, and must result in a Covered Loss within 365 days.
Only one principal sum (the largest applicable) is payable for a loss to the same limb due to or related to any one accident. Loss of sight means total and permanent loss thereof. Loss of limb means severance through or above the wrist or ankle joint.

Additional Seatbelt and Airbag Benefit

Under the AD&D option, your loved ones can collect an additional benefit amount up to $25,000 or 10% of your principal sum (whichever is less) if you suffer a covered loss of life as a result of injury sustained while riding in or operating an automobile and were found to be properly wearing a seatbelt.
An additional 5% will be paid if the automobile is equipped with an airbag. However, the total amount paid under the combined seatbelt and airbag benefit cannot exceed $25,000.

Valuable Living Benefit Provision “Accelerated Death Benefit”

The Accelerated Death Benefit option is available to help terminally ill insureds during a difficult and often financially challenging time. Under this provision, you may request one advance payment equal to 50% of your (or an insured dependent’s) in force life insurance to be paid while the terminally ill person is still alive. The request must be made at least 12 months prior to the insured person’s scheduled coverage termination age and the amount of insurance payable after the insured’s death will be reduced by this payment. (Premium contributions will not be reduced.)

This money can be used to help cover high prescription drug costs, medical bills, outstanding debts, to help pay for experimental treatments, the cost of modifications to your home, or for a family vacation – the choice is yours. 

To qualify, a terminally ill insured must provide New York Life Insurance Company with proof of terminal illness and anticipated life expectancy (12 months or less), as well as any other necessary medical information requested. For additional details and limitations, please see the Certificate of Insurance. 

Please note that receipt of Accelerated Death Benefits may affect your eligibility for public assistance programs and may be taxable. Prior to applying to receive such benefits, you should consult with the appropriate social services agency and seek the advice of a qualified tax advisor. 

Save with Volume Discounts on Higher Amounts of Insurance

If you or your spouse becomes insured for coverage amounts of $250,000 through $499,999, you’ll receive a volume discount. For amounts $500,000 through $999,000 of coverage, you’ll receive an even bigger discount; and a larger discount still for amounts of $1 million to $2 million.

Current 2024 Quarterly Rates –

Your Cost: The cost of the Member Term Life Insurance is based on the insured person’s attained age on the date coverage is issued and increases as he/she grows older. Premium contributions will vary depending on the number of units chosen.
Only non-smokers meeting the highest underwriting standards will qualify for Preferred rates. Other non-smokers may qualify for the higher Select or Standard rates. (Note: Smokers may only qualify for Standard rates.) Upon approval of your application, you will be notified of the rate classification for each approved person.

**The current 20% premium credit is not guaranteed and subject to change, however the Member Term Life Insurance for ACP members has returned premium credits for several years.

****If child coverage is requested, each eligible child will be covered for $5,000.

Residents of Montana: Male rates apply to all coverage, regardless of a person’s sex.

The premium contributions shown reflect the current rate and benefit structure. Premium contributions may be changed by New York Life Insurance Company on any premium due date and any date on which benefits are changed. However, your rates may change only if they are changed for all others in the same class of insurance under this group policy. For example, a class of insureds is a group of people all with the same issue age. Benefit amounts are not guaranteed and are subject to change by agreement between New York Life Insurance Company and the Trustees of the American College of Physicians, Inc. Insurance trust.

Waiver of Premium Benefit: If you become Totally Disabled for at least six consecutive months before age 60, your premiums for this policy will be waived until you are no longer disabled. 

Total Disability means an incapacity from an accidental injury or sickness which: completely and continuously prevents you from doing the material and substantial duties of any occupation for which you are reasonably qualified by education, training or experience, or; results in total and permanent loss in sight of both eyes; or the severance above the wrist or ankle of: (1) both hands; (2) both feet; or (3) one hand and one foot. 
The amount continued will be based on the option under which you were insured at the time your disability began, subject to the age decrease previously described. You may be asked to provide proof of you Total Disability from time to time.

Conversion Privilege: The Policy provides conversion privileges under certain circumstances of involuntary termination, as described in the Certificate of Insurance.

You Name Your Beneficiary: You may select any person, persons, trust or other legal entity as your beneficiary. If, at the time of your death, there are no surviving beneficiaries, benefits will be paid to the executor or administrator of your estate, or at the option of New York Life, to the surviving relatives in the following order of survival: spouse; children equally; parents equally; or brothers and sisters equally.

Ownership of Insurance: “Owner” means the person or entity with rights of ownership of this insurance as described in the Certificate of Insurance. If a transfer of ownership has been recorded by or on behalf of New York Life, or if initial ownership is by other than the member according to the information provided on the application, references throughout this Policy Information to “you” or “member” will mean “owner,” as applicable.

Effective Date: Insurance will take effect on the date your application is approved by New York Life Insurance Company provided the initial contribution is paid within 31 days after the date you are billed (send no money now), and any person to be insured is actively performing the normal activities of a person in good health of like age [NC residents: a person of like age] on the date of approval.

Any person who is not performing his/her normal daily activities as required will not become insured until the day he/she is performing such activities provided such date is within three months of the date insurance would have been effective, and the person is still eligible.

When Coverage Ends: Coverage will end when the insured person reaches age 75 (26 for children) or earlier if: (a) premium contributions are not paid when due, or (b) the insured requests to terminate insurance. In addition, dependent coverage will terminate when the member’s coverage terminates, or when the dependent eligibility requirements are no longer being met. Upon your death, coverage for your insured dependents may continue as described in the Certificate of Insurance.

In addition to the above, AD&D coverage ends when the insured member begins full-time active duty in the armed forces.

Renewal Payments And Claims: Once you are accepted into the Policy, you will have a 31-day grace period for your payment of renewal premium contributions. When you want to submit a claim, call or write the Administrator for claim forms.

Exclusions

For the Group Term Life Insurance coverage, the only exclusion is suicide. Benefits are paid for death from any other cause, at any time, anywhere in the world except suicide, within 12 months from the issue date, whether sane or insane.

The validity of any amount of your life insurance which has been in force for two years during an insured’s lifetime will not be contested except for insurance eligibility provisions and nonpayment of premium contributions.  

Exclusions and Limitations – AD&D

No benefit will be payable for any loss that occurs during or is due or related to: military service; your incarceration for or your participation in (except as a victim) an illegal occupation/activity or the commission of a crime; your voluntary intake of drugs, narcotics or alcohol (unless taken as prescribed by a physician); any declared or undeclared war or act thereof; or operating, riding in or descending from any aircraft except when riding as a passenger on a licensed, non- military aircraft; a physical or mental sickness or medical/surgical treatment thereof; or suicide or intentionally self-inflicted injury while sane or insane.

How New York Life Obtains Information and Underwrites Your Request For Member Term Life Insurance

In this notice, references to “you” and “your” include any person proposed for insurance. Information regarding insurability will be treated as confidential. In considering whether the person(s) in your request for insurance qualify for insurance , we will rely on the medical information you provide, and on the information you AUTHORIZE us to obtain from your physician, other medical practitioners and facilities, other insurance companies to which you have applied for insurance and MIB, LLC. (“MIB”). MIB is a not-for-profit organization of insurance companies, which operates an information exchange on behalf of its members. If you apply for life or health insurance coverage, a claim for benefits is submitted to an MIB member company, medical or non- medical information may be given to MIB, and such information may then be furnished by MIB, upon request, to a member company.

Your AUTHORIZATION may be used for a period of 24 months from the date you signed the application for insurance, unless sooner revoked. The AUTHORIZATION may be revoked at any time by notifying New York Life in writing at the address provided. Your revocation will not be effective to the extent New York Life or any other person already has disclosed or collected information or taken other action in reliance on it, or to the extent that New York Life has a legal right to contest a claim under an insurance certificate or the certificate itself. The information New York Life obtains through your AUTHORIZATION may become subject to further disclosure. For example, New York Life may be required to provide it to insurance, regulatory or other government agencies. In this case, the information may no longer be protected by the rules governing your AUTHORIZATION.

MIB and other insurance companies may also furnish New York Life, its subsidiaries or the Plan Administrator with non-medical information (such as driving records, past convictions, hazardous sport or aviation activity, use of alcohol or drugs, and other applications for insurance). The information provided may include information that may predate the time frame stated on the medical questions section, if any, on this application. This information may be used during the underwriting and claims processes, where permitted by law.

New York Life may release this information to the Plan Administrator, other insurance companies to which you may apply for life and health insurance, or to which a claim for benefits may be submitted and to others whom you authorize in writing, however, this will not be done in connection with test results concerning Acquired Immune Deficiency Syndrome (AIDS) or Human Immunodeficiency Virus (HIV). We may also make a brief report of your protected health information to MIB, but we will not disclose our underwriting decision.

New York Life will not disclose such information to anyone except those you authorize or where required or permitted by law. Information in our files may be seen by New York Life and Plan Administrator employees, but only on a “need to know” basis in considering your request. Upon receipt of all requested information, we will make a determination as to whether your request for insurance can be approved.

If we cannot provide the coverage you requested, we will tell you why. If you feel our information is inaccurate, you will be given a chance to correct or complete the information in our files. Upon written request to New York Life or MIB, you will be provided with non-medical information. Generally, medical information will be given either directly to the proposed insured or to a medical professional designated by the proposed insured. Your request is handled in accordance with the Federal Fair Credit Reporting Act procedures. If you question the accuracy of the information provided by MIB, you may contact MIB and seek a correction. California Consumer Privacy Act and California Privacy Rights Act: Privacy Policy for California Residents

  Taking into consideration the nature of the personal information and the purposes of the processing of your personal information, you may have the right to correct inaccuracies in such information. Upon submission to AGIA of a valid and verifiable request where your identity and authorization can be confirmed, AGIA will use all reasonable efforts to correct the inaccurate information.

  AGIA processes sensitive personal information only as necessary in performance of our services to ensure the security and integrity of your information or as otherwise specified under regulation. Because AGIA does not process your sensitive personal information for other purposes, there is not a mechanism for you to limit the processing of this information.

  AGIA does not engage in automated processing of personal information to make decisions that yield legal or other material consequence. Because we do not engage in this processing, AGIA does not provide a mechanism for you to limit our processing of personal information in this manner.

DEFINITIONS

The following defined terms are used in these Terms and Conditions:

“Payee” means the merchant or other entity to which you authorize a bill payment to be directed.

“Payee Account” means the billing account with the merchant or other entity to which you authorize a bill payment to be directed. It may be represented by an account number, policy number, access number or code, or other number used by the merchant to identify your account.

“Payment Instruction” means your instruction and authorization to make a bill payment to a Payee.

“Payment Account” means the bank account that you authorize A.G.I.A., Inc. and/or its affiliates to debit by electronic funds transfer when you make your bill payment using account information from your personal checking or savings account.

“Payment Card” means the credit card or debit card that you instruct A.G.I.A., Inc. and/or its affiliates to charge to pay your bill payment.

“Business Day” means Monday through Friday, excluding Federal Reserve holidays.

BILL PAYMENT SERVICE

When you initiate a Payment instruction, you authorize A.G.I.A., Inc. and/or its affiliates to charge your payment card or debit your payment account and remit funds to the Payee. If we are unable to obtain funds for a bill payment for any reason associated with your Payment Card or Payment Account (for example, there is not a sufficient balance available on your Payment Card or in your Payment Account to cover the transaction), then we will not be able to complete your bill payment transaction. If there is a problem in processing your Payment Instruction, A.G.I.A., Inc. and/or its affiliates may attempt to contact you, using the telephone number you have provided to A.G.I.A., Inc. and/or its affiliates.

If A.G.I.A., Inc. and/or its affiliates makes a bill payment to your Payee Account but is unable to obtain funds for that payment from your Payment Card or Payment Account, you agree that your bill payment to your Payee Account will be reversed.

You agree that from time to time the Payment services contemplated under this Agreement may be inaccessible or inoperable for any reason, including, without limitation: (i) equipment malfunctions; (ii) periodic maintenance procedures or repairs which may be undertaken from time to time; or (iii) causes beyond A.G.I.A., Inc. and/or its affiliates control or which are not reasonably foreseeable to A.G.I.A., Inc. and/or its affiliates.

OUR LIABILITY FOR FAILURE TO COMPLETE TRANSACTIONS

A.G.I.A., Inc. and/or its affiliates will use commercially reasonable efforts to process your bill payments in accordance with your Payment Instructions. However, A.G.I.A., Inc. and/or its affiliates shall incur no liability if a bill payment is not made in a timely manner or if it is unable to complete any payments initiated by you through the A.G.I.A., Inc. and/or its affiliates Bill Payment Service because of the existence of any one or more of the following circumstances:

  • Sufficient funds are not available through your Payment Account or your Payment Card;
  • Failure of any Payee to account correctly for or credit the payment in a timely manner, or otherwise mishandle or delay the payment;
  • The A.G.I.A., Inc. and/or its affiliates Bill Payment Service is not working properly and you know or have been advised by A.G.I.A., Inc. and/or its affiliates about the malfunction before you execute the transaction;
  • You do not provide A.G.I.A., Inc. and/or its affiliates with all required information to complete the bill payment, such as your correct name, telephone number, or your complete and correct Payee account information; or
  • Circumstances over which A.G.I.A., Inc. and/or its affiliates has no control including, but not limited to large-scale technical malfunctions, including, but not limited to loss of access to the Internet or loss of access to the Federal Reserve System, prolonged outages of phone lines, electricity or similar infrastructure, acts of God, war, riot, civil disobedience or similar events of insurrection, governmental or court orders, regulatory or legislative changes by any local, state or federal governmental agency, strikes, work stoppages, or other similar occurrences or circumstances.

LIMITATION OF LIABILITY AND YOUR EXCLUSIVE REMEDY

In the absence of any of the exclusions set forth above, it shall be A.G.I.A., Inc. and/or its affiliates responsibility to correct any transactions not completed in accordance with your Payment Instructions. In the event that A.G.I.A., Inc. and/or its affiliates is unable to correct the transaction, we shall refund the amount of funds received from you and this refund shall constitute your sole remedy. In no event shall A.G.I.A., Inc. and/or its affiliates liability to you ever exceed the amount of funds that we receive from your Payment Account or Payment Card for a bill payment. The foregoing shall constitute A.G.I.A., Inc. and/or its affiliates entire liability and your exclusive remedy. The use of this payment service is at your sole risk. In no event shall A.G.I.A., Inc. and/or its affiliates be liable for any other loss, injury, or damages, whether direct, indirect, special, incidental, consequential, or exemplary, including lost profits (even if advised of the possibility thereof) arising in any way out of your use of the bill payment service. Except as otherwise explicitly set forth herein, we specifically disclaim and you understand that we make no warranties or representations of any kind, expressed or implied and the same are hereby excluded from these terms and conditions and all transactions contemplated hereby. Some states prohibit the exclusion or limitation of incidental or consequential damages, thus this limitation of liability may not apply to you.

USER AUTHENTICATION AND SECURITY

A.G.I.A., Inc. and/or its affiliates Bill Payment Service has been designed in compliance with the Payment Card Industry (PCI) data security standards so that no one can access your Payment Account without proper user authentication. If you use this site, you are responsible for maintaining the confidentiality of your personal information, including your Payment Account number, and for restricting access to your computer, and you agree to accept responsibility for all activities you initiate or that you authorize to be initiated or that are authorized utilizing your payment account or from your computer. You are solely responsible for all bill payments you authorize using Bill Payment Service. If you permit other persons to use your Bill Payment Service or your bill payment authentication responses or other means to initiate a Payment Instruction on your behalf, you are solely responsible for any transactions they authorize.

If you believe that your authentication information or other personal information have been lost or stolen or that someone may attempt to use your Bill Payment Service without your consent, you must notify A.G.I.A., Inc. at once by calling (800) 424-5181 during normal customer service hours.

CHARGES

Transaction charges, if any, for using the Bill Payment Service may vary depending upon the Payee and permissible charges allowed in a particular state. These charges will be disclosed to you prior to your Payment Instruction. By submitting your Payment Instruction, you agree to pay these transaction charges. There may be a charge for additional transactions and other optional services. You agree to pay such charges and authorize A.G.I.A., Inc. and/or its affiliates to charge your designated Payment Card or debit your Payment Account for these amounts and any additional charges that may be incurred by you. You are responsible for any and all fees and charges that you may incur.

GOVERNING LAW

These Terms and Conditions shall be governed by the laws of the state of California without regard to that state’s conflicts of laws provisions and by applicable Federal laws and regulations.

WAIVERS

No delay or omission by us in exercising any rights or remedies under these Terms and Conditions will impair such right or remedy or be construed as a waiver of any such right or remedy. If we exercise any right or remedy, in whole or in part, that exercise will not prevent us from any further or future exercise of such right or remedy or any other right or remedy. No waiver will be valid unless in writing signed by us.

DISCLOSURES & CONSENT TO ELECTRONIC TRANSACTIONS / ELECTRONIC SIGNATURE

  1. By entering this website and agreeing to be bound by this Agreement, you are providing your affirmative consent to the use of an electronic signature to authenticate the insurance transaction in electronic form. You understand and agree that the insurance company will rely on your electronic signature to process and effect insurance transactions.
  2. You acknowledge that you understand that you are not obligated to enter into transactions electronically and that you have a right to conduct insurance transactions in paper format if you wish. By entering this website and agreeing to be bound by this Agreement, you affirmatively consent to conduct transactions in electronic form. If you wish to conduct insurance transactions in paper form, please contact your association/policyholder or its representatives. There is no charge to you for requesting a paper transaction.
  3. When you have successfully entered your enrollment information and have selected “I Accept” on the website confirmation page, you will be given the opportunity to print a paper copy of your insurance elections free of charge. Please print a paper copy of your insurance elections for your records because if you will not be able to access your enrollment information on the Site after your enrollment. If you wish to confirm or make changes to your coverage or beneficiary elections after your enrollment, you must contact the A.G.I.A., Inc. and/or its affiliates Endorsed Insurance Program Office of Administration. You understand and agree that requests for change submitted to Carrier or the Site will not be valid or take legal effect after the end of your enrollment.
  4. Once you have given consent for an electronic transaction, you may withdraw your consent only if Carrier has not taken action in reliance on your consent. To update your contact information or to withdraw your consent to an electronic transaction, you may contact your Administrator by calling (800) 424-5181. There is no fee charged for such requests. However, your withdrawal of consent shall not diminish the legal effectiveness or enforcement of any transaction agreed to prior to your withdrawal of consent. If you withdraw consent after your enrollment, your insurance will remain effective until your request for cancellation is received by the group policyholder or its representative.
  5. You agree that your electronic signature authorizes The Underwriting Carrier(s) or its authorized representatives:
    1. to process this insurance transaction at your request and any future transactions that may be needed to administer and help keep in force your coverage under the insurance policy. For example, we will rely on your electronic signature to authorize us to process your request for insurance coverage(s) and beneficiary designation(s) made during your enrollment.
    2. to process, as applicable:
      1. a billing transaction, including but not limited to: processing a payment by credit card using the credit card number you have provided; processing an electronic bank draft using the checking account number you have provided; or sending you a billing statement at the address you have provided; or
      2. a billing transaction with your financial institution to deduct the appropriate amount from your account.
    3. to communicate with you by mail, telephone or electronically by sending to you communications including, but not limited to, any and all types of electronic communication by email, fax, mail and telephone.
  6. After you enroll, you understand and agree that you will not have access to your electronic enrollment record. You may print a confirmation of your enrollment election at the time of enrollment, or you may contact your Administrator for confirmation after your enrollment period ends.

USER REPRESENTATION

You represent and warrant to A.G.I.A., Inc. and/or its affiliates that: (a) you over the age of eighteen (18) and have the power and authority to enter into an perform your obligations under this Agreement; (b) all information provided by you to A.G.I.A., Inc. and/or its affiliates is truthful, accurate and complete; (c) you are the authorized signatory of the credit or charge card provided to pay fees contemplated herein; (d) you shall comply with all of the terms and conditions of this Agreement; and (e) you have provided and will continue to provide accurate and complete registration information, including, without limitation, your legal name, address and telephone number.

A.G.I.A., Inc. and/or its affiliates provide information and services on this website, and all layouts, materials, designs, and images on this website are copyrighted or proprietary to A.G.I.A., Inc., its affiliated companies and/or third party service providers. You understand and agree that the information on Carrier’s insurance products as described in this website is not complete and does not change or affect the insurance policies as actually issued. Although you have been provided with a description of benefits, you understand that only the insurance policy issued to the policyholder (your association) can fully describe all of the provisions, terms, conditions, limitations and exclusions of your insurance coverage. In the event of any difference between the information on the website and services, the benefit highlights, and the insurance policy, the terms of the insurance policy apply.

You understand and agree that insurance will not be valid or in force with respect to you or your dependents if any such person is not eligible in accordance with the terms of the group policy issued to your association/policyholder. You acknowledge and agree that if group participation requirements are not met, the insurance policy will not be issued and the elected coverage(s) will not be in force.

You understand that this website and the Payment services contemplated hereunder may be performed by third-party vendors.

If any provision of this Agreement is determined to be illegal or unenforceable, then such provision will be enforced to the maximum extent permissible and the other provisions will remain fully effective and enforceable.
